Job summary

VitalCaring Group is seeking a Licensed Practical Nurse (LPN/LVN) for Home Health to provide direct, compassionate care in patients’ homes within an assigned territory. You will implement care plans under RN supervision and deliver skilled nursing services to support recovery and independence.

The role emphasizes hands-on care, patient education, and collaboration with an interdisciplinary team, with a flexible field-based schedule and strong focus on quality outcomes.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus)
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while recognizing and reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.

Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.
