Key Responsibilities:

• Provide skilled nursing care under the supervision of a Registered Nurse

• Deliver compassionate, patient-centered care that aligns with our mission

What We’re Looking For:

• Licensed Practical Nurse (LPN) with a minimum of one year of Home Health experience
• OASIS knowledge & competency (a plus!)
• Experience with Home Care Home Base (HCHB) is a bonus
• Strong communication and organizational skills
• Ability to work independently and as part of a team
• Detail-oriented, self-motivated, and passionate about patient care
