VitalCaring Group in Gretna, LA is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for Home Health. The role involves providing direct patient care in patients' homes under RN supervision, implementing care plans, and supporting recovery and independence.

You will deliver skilled nursing services, monitor conditions, educate patients and caregivers, document care in EMR, and collaborate with an interdisciplinary team.

Qualifications

• Active LPN/LVN license in the state of practice.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med‑Surg, or Acute Care a plus).

Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.

What You Bring

• Active LPN/LVN license in the state of practice
• Graduate of an accredited practical/vocational nursing program
