VitalCaring Group is seeking a licensed LPN/LVN to provide direct home health care in patients’ homes. You will deliver skilled nursing services, monitor conditions, educate caregivers, and support care plans under RN supervision.

You’ll work field-based with a flexible schedule, collaborating with an interdisciplinary team to ensure high-quality, compassionate care and optimal patient outcomes in the comfort of home.

Qualifikationen

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one (1) year nursing experience preferred (Home Health, Med‑Surg, or Acute Care a plus)
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while recognizing and reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.
