Job summary

Safe Life Home Health Care is seeking an experienced Licensed Practical Nurse (LPN) to deliver in-home skilled nursing under RN/physician direction. You will perform medication administration, wound care, patient teaching, and safety monitoring while ensuring timely and accurate documentation.

The role requires strong communication and the ability to collaborate with RN Case Managers and the interdisciplinary team to achieve quality outcomes in a home health setting.

Qualifications

• Graduate of an accredited program; Active Illinois LPN license; BLS/CPR.
• 1+ year nursing experience; Medicare home-health experience preferred.
• EMR experience; WellSky a plus; strong communication and customer-service skills.
• Valid driver’s license, auto insurance, and reliable vehicle.

Responsibilities

• Perform LPN interventions during scheduled home visits and report changes in condition.
• Implement the Plan of Care; reinforce disease-specific and medication teaching.
• Obtain vitals, administer treatments, and perform wound/skin care per order.
• Document visits in WellSky within required timeframes; maintain supplies and infection prevention.
• Collaborate with RN Case Managers and interdisciplinary team to achieve quality outcomes.
