Job Summary

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the care plan, administering treatments, and collaborating with team members to achieve positive patient outcomes.

Essential Responsibilities

• Provide direct patient care as defined by the State Nurse Practice Act.
• Implement the care plan initiated by the clinician.
• Document care accurately and timely.
• Assess and educate patients and families about diagnoses and care plans.
• Coordinate home health services and report needs to the appropriate clinician or supervisor.
• Use equipment and supplies efficiently.
• Engage in professional growth and development.
• Perform additional duties as assigned by the RN.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Preferably one year of nursing experience, especially in community, home health, or medical/surgical settings.
• Current license as a practical or vocational nurse in the state.
• CPR certification.
• Adherence to professional standards and practices.
• Good communication and organizational skills.
• Must have a valid driver’s license, insured vehicle, and good driving record.
