Job summary

Professional Case Management (PCM) is hiring LPNs/LVNs to provide in-home health services across New Mexico. Choose days, nights, and weekends to fit your schedule and manage one-on-one patient care in the home setting.

Qualifications

• Graduate of a state-approved school of professional nursing.
• Current, unrestricted LPN/LVN license in NM.
• Prefer minimum 1 year experience in acute care; training considered for new grads.
• Current CPR certification and TB test.

• Provide in-home nursing care and assessments.
• Develop and implement patient care plans.
• Educate patients and families on health maintenance.
• Maintain safety and confidentiality in clients' homes.
