Job summary

Visiting Nurse Health System is seeking a Licensed Practical Nurse to join our Home Health team in the Conyers/Covington area. This non-exempt, full-time role supports patients with nursing care, education, and comfort, Monday through Friday. The position is headquartered at our Ravina office in Dunwoody, GA.

We value respect, integrity, excellence, stewardship, and compassionate care. An active GA LPN license, IV therapy training, and 1–2 years in Medicare-certified home health are preferred.

Qualifications

• LPN graduate from an accredited nursing program.
• IV training or IV therapy certification.
• Experience with advanced wound therapies preferred.
• Georgia State Board of Nursing active license.
• 1–2 years in Medicare-certified home health agency preferred.
• 2 years experience as an LPN.
• Valid driver’s license with auto insurance and reliable vehicle.

Responsibilities

• Provide nursing care, emotional support, and patient/family education.
• Provide nursing services as ordered by physician and under RN supervision.
• Report subjective and objective patient findings to the RN.
• Obtain physician’s orders and ensure orders exist for each visit.
• Provide necessary skills to report changes and document care to ensure continuity.
• Follow infection control standards per VNHS policy and procedures.

In this position...

• Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
• Provides nursing services as ordered by the physician and under the direction of the RN.
• Reports subjective and objective patient findings to the RN.
• Under the direction of the RN, obtains physician’s orders and ensures orders exist for each visit made,
• Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care.
• Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.

Do you have....

• LPN Graduate from an accredited school of nursing or program
• Completion of an IV training program or certification in IV therapy
• Experienced in advanced wound therapies
• Current license by the Georgia State Board of Nursing.
• Minimum of one - two (1-2) years’ experience in a Medicare-certified home health agency is preferred.
• Minimum of two (2) years as an LPN
• Current valid Driver’s License, proof of auto insurance, and a reliable vehicle

Salary Range: $61,634.56-$92,451.84
