Job summary

Visiting Nurse Health System (VNHS) is seeking a Licensed Practical Nurse for Home Health to join our team in the East Point and College Park area. The role is full-time and non-exempt, with Monday through Friday visits across the specified region.

We value a compassionate, educated nurse who can educate patients and families, follow physician orders, and maintain accurate documentation while upholding VNHS infection-control standards.

Qualifications

• LPN graduate from an accredited school of nursing.
• Completion of an IV training program or IV therapy certification.
• Experience with advanced wound therapies.
• Current Georgia State Board of Nursing license.
• 1–2 years of Medicare-certified home health agency experience preferred.
• Minimum of 2 years as an LPN.
• Valid driver’s license with proof of auto insurance and reliable vehicle.

Responsibilities

• Provide nursing care, emotional support, patient and family education for Home Health patients.
• Provide nursing services as ordered by physician and under RN direction.
• Report patient findings to the RN (subjective and objective).
• Obtain physician orders under RN guidance and ensure orders exist for each visit.
• Document care and changes to ensure continuity of care.
• Adhere to infection control standards per VNHS policies.

In this position...

• Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
• Provides nursing services as ordered by the physician and under the direction of the RN.
• Reports subjective and objective patient findings to the RN.
• Under the direction of the RN, obtains physician’s orders and ensures orders exist for each visit made,
• Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care.
• Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.

Do you have....

• LPN Graduate from an accredited school of nursing or program
• Completion of an IV training program or certification in IV therapy
• Experienced in advanced wound therapies
• Current license by the Georgia State Board of Nursing.
• Minimum of one - two (1-2) years’ experience in a Medicare-certified home health agency is preferred.
• Minimum of two (2) years as an LPN
• Current valid Driver’s License, proof of auto insurance, and a reliable vehicle
  

Salary Range: $61,634.56-$92,451.84
