Interim HealthCare in Anderson, SC is seeking a Home Health Licensed Practical Nurse (LPN) to provide skilled, compassionate care in patients' homes, helping them recover and live independently.

You will work with a team, take vitals, document progress, teach self-care, and coordinate discharge planning. Competitive pay starting at $33 per visit plus mileage, with health coverage and 401(k).

Formación

• Graduate of a practical nursing program.
• Active LPN/LVN license in South Carolina.
• At least 1 year of LPN/LVN experience.

WhatYou’llDo

As a Home Health Licensed Practical Nurse (LPN), you will:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN.
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care.
• Take patient vitals, do assessments, document progress and report changes to RN supervisor.
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques.
• Educate family on the patient’s plan of care, medication and home safety.
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge.

What You’ll Need

• Graduate of a practical (vocational) nursing program for LPN/LVN
• Current license as an LPN/LVN in the South Carolina.
• At least one-year or 1200 hours of experience as an LPN/LVN.
