Job summary

Interim HealthCare is seeking a Home Health Licensed Practical Nurse (LPN) to provide compassionate, skilled care in patients' homes across South Carolina. You will support recovery, chronic disease management, safety at home, and family education, under RN supervision.

This role emphasizes trusted relationships and coordinated care. You’ll work with a team of physicians, RNs, CNAs and aides to deliver quality outcomes, enjoy competitive pay per visit starting at $33, flexible scheduling, 1:1

Qualifications

• Graduate of a practical nursing program for LPN/LVN.
• Current SC LPN/LVN license.
• At least 1 year or 1200 hours of LPN/LVN experience.

Responsibilities

• Provide home-based care to patients under RN supervision.
• Collaborate with physicians, RNs, CNAs and aides to deliver care.
• Take patient vitals, perform assessments, document progress.
• Assist RN with plan of care and teach self-care techniques.
• Educate family on the plan of care, medications and home safety.
• Coordinate patient care and discharge planning with interdisciplinary team.

WhatYou’llDo

As a Home Health Licensed Practical Nurse (LPN), you will:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN.
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care.
• Take patient vitals, do assessments, document progress and report changes to RN supervisor.
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques.
• Educate family on the patient’s plan of care, medication and home safety.
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge.

What You’ll Need

• Graduate of a practical (vocational) nursing program for LPN/LVN
• Current license as an LPN/LVN in the South Carolina.
• At least one-year or 1200 hours of experience as an LPN/LVN.
