Job summary

MedStar Health’s Washington Hospital Center is seeking a Full Time Licensed Practical Nurse for the Washington DC area with home care experience. You will support care plans for patients in their homes and help families achieve health goals, with one weekend requirement per month.

The Home Care team emphasizes patient-centered decisions and collaborates with nurses, therapists, social workers, and aides to provide 24/7/365 support across cases.

Qualifications

• Graduation from an accredited Practical Nursing program is required.
• 1-2 years recent clinical experience in an acute care setting required; home care experience preferred.
• LPN license in DC/MD/VA valid upon hire; valid driver’s license and CPR certification required within 90 days.

Responsibilities

• Provide skilled home health care to patients and families under RN supervision.
• Support care plans and assist patients and caregivers toward improved health and independence.
• Adhere to agency policies and maintain detailed documentation of care provided.

Minimal Qualifications

Education

• Graduation from an accredited program of Practical Nursing required

Experience

• 1-2 years recent clinical experience in an acute care setting required
• Home care / Home health experience preferred

Licenses and Certifications

• LPN - Licensed Practical Nurse - State Licensure Valid LPN license in the District of Columbia Maryland or Virginia or any combination as required based on work location(s) Upon Hire required and
• DL - Valid State Driver's License in good standing with a dependable vehicle for transportation Upon Hire required and
• CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and
• Additional unit/specialty certifications may vary by department or business unit.
