Position Summary: Home Care is in high demand and we are looking forexperienced and caring LPNs to join our team at Home Health Care Incorporated!Successful applicants will have a proven track record as an LPN, with home careexperience a huge benefit but not required. Responsibilities:

• Provide patientcare according to client’s care plan and needs
  
• Perform woundcare treatments and assist clients with medication management
  
• Teach theclient and/or family members about care and support skills
  
• Communicateregularly with RN Case Managers, Supervisors, and the Care Team
  

Qualifications:

• Licensed as aPractical Nurse / LPN in the state of Minnesota
  
• Driving isrequired; must have a reliable vehicle with current insurance and a validDriver's License
  
• Demonstratedability to work autonomously
  
• Prior Nursingexperience in a TCU, Hospice, or Home Health setting
  
• Experience withElectronic Medical Records (EMR) software is preferred
