Job summary

Seven Hills Health & Rehabilitation Center in Tallahassee, FL is hiring full-time Licensed Practical Nurses (LPNs) for 11:00 PM - 7:00 AM shifts. Join a 156-bed skilled nursing facility dedicated to compassionate, individualized care and collaborative practice with physicians, residents, and families.

Responsibilities include admissions, transfers, medication administration, infection control, and coordinating with the interdisciplinary team to deliver high-quality care while following state

Qualifications

• LPN license valid in the state
• CPR Certification required
• Two years nursing experience preferred

Responsibilities

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Infection control and center policies following procedures
• Collaborating with interdisciplinary team to ensure best outcomes
• Identify and report abnormal changes in residents’ condition per center policy
• Work under supervision in line with state Nurse Practice Act and facility policies
• Deliver nursing care for long-term or rehabilitative residents
• Collect patient data, observe, and report pertinent information
• Implement and evaluate patient care plan per regulations
• Direct and supervise care by other nursing personnel when appropriate
• Maintain documentation and equipment knowledge
• Ensure confidentiality and patient rights

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
• CPR Certification required
• Two Years prior nursing experience preferred
