Job summary

Interim HealthCare - North Charleston, SC is seeking a Home Health LPN for full-time employment. Enjoy flexible assignments and the ability to tailor your schedule to fit family and career goals while delivering high-quality patient care.

You will provide home-based nursing under RN supervision, collaborate with a multidisciplinary team, monitor vitals, document progress, and educate patients and families on plans of care and safety. Training and CEUs are available.

Qualifications

• Graduate of an accredited vocational nursing program with active SC/Compact LVN license.
• Minimum 1 year of LPN experience, ideally in home health care.
• CPR certification and valid driver's license with car insurance.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based care to patients under RN supervision.
• Collaborate with physicians, RNs, CNAs and aides to deliver care.
• Take vitals, assess, document progress, and report changes.
• Educate patients and families on plan of care and safety.
• Coordinate discharge planning with interdisciplinary team.

As a Home Health Licensed Vocational Nurse, heres a big-picture view of what youll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN


• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care


• Take patient vitals, do assessments, document progress and report changes to RN supervisor


• Assist RN in carrying out patients plan of care and teaching patient self-care techniques


• Educate family on the patients plan of care, medication and home safety


• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Licensed Vocational Nurses:

• Graduate of an accredited vocational nursing program and active LVN license in South Carolina/Compact License.


• Minimum of 1 year of LPN experience, ideally in home healthcare


• CPR certification


• Valid car insurance and Driver's License


• Knowledge of state and federal home health regulations


• Clinical proficiency in nursing, strong communication skills, and compassion
