Job summary

Signature HealthCARE of Norfolk seeks an experienced LPN/LVN to deliver direct nursing care in a 169-bed facility. You will supervise nursing assistants, administer medications, and contribute to care plans with an interdisciplinary team.

The role requires active LPN/LVN licensure, 1+ year of nursing experience (long-term care preferred), and strong clinical judgment. This is a full-time day shift with benefits and a $7,500 sign-on bonus where applicable.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents' physical and emotional status; notify attending physician and responsible party of any change.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

How you Will make a Difference
