Job summary

Signature HealthCARE in Norfolk, VA is hiring a licensed practical/vocational nurse to provide direct resident care, manage meds, and supervise nursing assistants. You’ll participate in care planning, document diligently, and coordinate admissions and discharges in a team-oriented setting.

The role requires an active LPN/LVN license, 1+ year LTC-leaning nursing experience, and familiarity with EHR systems. Strong communication and independent judgment are essential.

Qualifications

  • Current, active LPN/LVN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
  • Experience in a supervisory or charge nurse role preferred.
  • Strong clinical observation, medication administration, and documentation skills.
  • Knowledge of Federal and State long-term care regulations.
  • Effective verbal and written communication skills.
  • Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift per policy.
  • Monitor and observe residents’ status; notify physician and responsible party of changes.
  • Develop, implement, and review individualized resident care plans with the team.
  • Complete accurate and timely clinical documentation per policy.
  • Coordinate resident admissions, transfers, and discharges.
  • Participate in QA, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing.

How You Will Make a Difference

  • Coordinate resident admissions, transfers, and discharges according to established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.
