Job summary

Signature HealthCARE of Norfolk is actively seeking an LPN/LVN to provide direct nursing care in a 169-bed facility. This full-time, day-shift role offers a $7,500 sign-on bonus and opportunities to supervise nursing staff while ensuring high-quality resident care and adherence to regulations.

Responsibilities include medication administration, ADL support, care plan execution, and accurate charting. Qualifications include active licensure, 1+ year of nursing experience, and familiarity with EHR

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
