Summary

We are seeking a caring and compassionate Licensed Practical Nurse (LPN) who will provide excellent direct care to our residents on a daily basis. The incumbent will perform assigned nursing procedures for the comfort and well‑being of residents, such as assisting in the admission of new residents, assisting with activities of daily living, administering specified medications, supervising non‑licensed nursing staff, and participating in the evaluation of non‑licensed staff.

Duties and Responsibilities
  1. Demonstrates excellent decision‑making skills and understands the impact of those decisions.
  2. Sets and manages priorities.
  3. Accepts calls from staff during nights and weekends.
  4. Meets with incoming and outgoing LPNs to discuss residents’ conditions and shift incidents.
  5. Ensures clear, accurate, and timely communications.
  6. Counts controlled substances with the outgoing nurse and assumes responsibility for accuracy before accepting keys.
  7. Checks, orders, and administers prescribed medication with 100% accuracy.
  8. Ensures correct dosage by never pre‑pouring medications.
  9. Demonstrates knowledge of important medication interactions and side effects.
  10. Ensures medication cart is locked at all times.
  11. Obtains pertinent information and completes all required forms as established by policy, including upon resident admission.
  12. Completes and maintains ECC and LNS logs, service plans, progress notes, monthly summaries and follows healthcare providers’ orders.
  13. Assists in completing evaluations when needed for residents.
  14. Provides hands‑on care as needed to meet the residents’ needs.
  15. Works with ADORS to review and investigate incident reports, recommending appropriate action and follow‑up to the DORS/executive director.
  16. Understands and adheres to ALF regulations pertaining to assisted living health‑care laws.
  17. Maintains a safe and secure work environment free of hazards that could cause harm or falls to residents, families, visitors and associates.
  18. Practices good body mechanics and safe working habits.
  19. Follows established policies and procedures in emergency situations.
  20. Gives associates in nursing daily feedback on their performance.
  21. Participates in associates’ performance evaluations when requested by the DORS.
  22. Ensures associates participate in activities provided to residents as described in the activity calendar.
  23. Responds promptly and courteously to residents’ requests for assistance, including phone calls and family or visitor requests.
  24. Maintains a safe, clean, and orderly environment for all residents.
  25. Supports a dignified and caring atmosphere for residents, families, visitors, and associates.
  26. Wears clean and appropriate dress per established policy.
  27. Complies with HIPAA regulations.
  28. Considers essential personnel during emergencies.
  29. Takes initiative in work, seeks additional information, and offers solutions or suggestions.
  30. Performs all other reasonable duties as assigned or requested.
  31. Willingly takes on special projects or assignments outside of regular duties.
Supervisory Responsibilities

Directly supervises employees in the Resident Services/Nursing Department. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Physical Demands

The employee must regularly stand, walk, use hands, reach with hands and arms, and talk or hear. Frequently required to stoop, kneel, crouch or crawl; occasionally to sit, climb, or balance, and taste or smell. May occasionally lift or move up to 50 pounds.

Qualifications
Language Ability

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Math Ability

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent, and to draw and interpret bar graphs.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills

Knowledge of word processing software and electronic medical records.

Education/Experience

High school diploma or GED; and one to two years of related experience and/or training; or equivalent combination of education and experience.

Certificates and Licenses

Valid state‑specific nursing license.
