Job summary

Glen Haven Health & Rehabilitation in Northport, AL is seeking a full-time Licensed Practical Nurse (LPN) for the 3:00 PM–11:00 PM evening shift. Base pay ranges from $25.00 to $29.55 per hour, based on experience, with evening shift differentials. The LPN will provide direct nursing care in a skilled nursing and long-term care setting, collaborate with RNs and physicians, supervise CNAs, and document care.

Qualifications

• Active LPN license in Alabama in good standing.
• CPR/BLS certification.
• Experience in skilled nursing or geriatric care preferred but not required.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating with the nursing team.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing, rehabilitation, and long-term care setting.
• Administer medications, treatments, and nursing interventions according to physician orders.
• Assess, monitor, and document resident conditions.
• Recognize changes in condition and communicate clinical concerns promptly.
• Maintain accurate and timely electronic nursing documentation.
• Collaborate with Registered Nurses (RNs), physicians, therapists, and other members of the interdisciplinary care team.
• Supervise and support CNAs to ensure quality resident care.
• Communicate effectively with residents and families regarding care needs.
• Promote resident safety, comfort, dignity, and quality of life.
• Follow infection prevention, safety, and regulatory standards.
