Job summary

Guided Steps NJ, Corp. is seeking Licensed Practical Nurses (LPNs) to provide skilled nursing in clients' homes across Northern New Jersey under RN supervision.

This per diem role offers flexible scheduling, competitive hourly pay of $35-$40 based on experience, with required NJ LPN license, CPR/BLS, and prior home care or medically complex care experience. Travel to service locations and onboarding/background checks required; benefits include PTO, health insurance, and holiday pay.

Qualifications

• Current, unrestricted New Jersey LPN license.
• Graduation from an accredited practical nursing program.
• Experience supporting individuals with intellectual and developmental disabilities is preferred.
• Current CPR/BLS certification is required.
• Minimum of two years direct nursing care experience as an LPN.
• Experience in home care or community-based settings is preferred.

Responsibilities

• Administer prescribed medications and nursing treatments.
• Provide G-tube care, enteral feedings, and medication administration via feeding tubes.
• Perform wound care, dressing changes, and skin monitoring.
• Provide catheter care and other prescribed nursing procedures.
• Monitor seizure-related care needs and respiratory care as trained.
• Monitor vital signs and health status; maintain accurate MARs and documentation.
• Collaborate with caregivers, healthcare providers, and supervising RN.

Position Summary

Guided Steps, an approved DDD provider, is seeking compassionate, reliable, and experienced Licensed Practical Nurses (LPNs) to provide skilled nursing care to individuals with intellectual and developmental disabilities in their homes. The LPN will provide direct nursing services under the direction and supervision of the agency's Registered Nurse (RN). Each individual has unique needs and requires personalized care and support. The following outlines general responsibilities, which may vary based on each individual's specific needs, goals, and level of support required.

Essential Responsibilities may include:

• Administer prescribed medications and skilled nursing treatments.
• Provide G-tube care, enteral feedings, and medication administration through feeding tubes.
• Perform wound care, dressing changes, and ongoing skin monitoring.
• Provide catheter care and other prescribed nursing procedures.
• Monitor seizure-related care needs.
• Provide respiratory care, including suctioning when appropriately trained and authorized.
• Monitor vital signs, blood glucose levels, and changes in health status.
• Maintain accurate MARs and nursing documentation.
• Collaborate with caregivers, healthcare providers, and the supervising RN.

Required Qualifications

• Current, unrestricted New Jersey Licensed Practical Nurse (LPN) license.
• Graduation from an accredited practical nursing program.
• Experience supporting individuals with intellectual and developmental disabilities.
• Current CPR/BLS certification.
• Minimum of two (2) years of experience providing direct nursing care as an LPN required.
• Experience in home care, private duty nursing, subacute rehabilitation, long-term care, or care of medically complex individuals required.
• Experience with medication administration required.
• Experience with G-tube care and enteral feedings required.
• Experience caring for individuals with complex medical needs.
• Strong clinical observation, documentation, and communication skills.
• Ability to work independently within the LPN scope of practice and under appropriate RN supervision.
• Reliable transportation and ability to travel to assigned service locations.
• Ability to successfully complete all agency onboarding, credentialing, and background screening requirements.

Preferred Qualifications

• Experience with seizure management, respiratory care, suctioning, wound care, and catheter care.
• Familiarity with individualized nursing care plans and medically complex populations.
• Previous experience providing services in a home or community-based setting.
