Job summary

Avera McKennan Hospital in Sioux Falls, SD seeks a Licensed Practical Nurse (LPN) for day shift, regular hours, 40 per week. The LPN will provide patient-centered nursing care under supervision and collaborate with a multidisciplinary team to plan and implement care.

Responsibilities include assessments, implementing interventions, care plan development, and patient education. You must hold an active LPN license in the state and communicate effectively in a team environment.

• Participates with other health care providers in the development and modification of the patient-centered health care plan.
• Implements nursing interventions within a patient-centered health care plan.
• Assists in the evaluation of responses to interventions.
• Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
• Advocates for the best interest of the patient.
• Communicates and collaborates with patients and members of the health care team.
• Assists with health counseling and teaching.
• Delegates and assigns nursing interventions to implement the plan of care.

A Brief Overview

The LPN practices under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which he/she is accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.
