The Licensed Practical Nurse's primary role is to provide nursing care for patients experiencing common, well-defined health problems. The LPN functions as a member of the multidisciplinary healthcare team, utilizing the nursing process to implement patient care. Services are performed under the supervision of a registered nurse but require all nursing skills to make safe, appropriate, and independent decisions within the scope of LPN practice. Duties are carried out in accordance with Joint Commission, Federal and State regulations, Oceans' Mission, policies, procedures, and Performance Improvement Standards.

Essential Functions:

  1. Deliver quality nursing care to an assigned group of patients by collecting data and identifying deviations from normal; assist with reassessment of patients according to assignments, needs, and unit protocols under the direction of the registered nurse.
  2. Accurately and thoroughly document all findings in the patient record and identify expected outcomes based on assessment data; effectively communicate pertinent assessment findings with other healthcare team members.
  3. Implement nursing care interventions identified in the individualized multidisciplinary treatment plan using accepted standards of practice in collaboration with other healthcare providers; perform nursing procedures/treatments as prescribed by physicians or licensed independent practitioners (LIP).
  4. Conduct bedside laboratory tests, wound care, and assist with activities of daily living (ADLs) to promote patient comfort and well-being.
  5. Support the patient's ability to cope, adapt, and resolve issues related to illness and stressors.
  6. Provide age-appropriate, culturally sensitive communication and interventions, documenting any special needs.
  7. Administer medications safely according to facility policies and physician orders; assess patients before and after medication administration; communicate and document doses accurately; educate patients about their medications.
  8. Follow up on patient care responsibilities; communicate any incomplete assignments or patient needs with other disciplines to ensure continuity of care; transcribe orders accurately and perform chart audits as per facility guidelines.
  9. Utilize nursing knowledge to meet the needs of the facility, patient population, and staff; leverage nursing resources to promote evidence-based practice.
  10. Guide, educate, and provide leadership as a preceptor to new staff; perform other duties and projects as assigned.
