Job summary

Pleasant Hills Community Living Center is seeking a Licensed Practical Nurse LPN to join our growing team. This full-time position offers day 7am–3pm and evenings 3pm–11pm shifts, with competitive pay and benefits.

As an LPN, you will administer medications, perform ADL care, conduct rounds, and communicate with residents, families, and physicians. You will maintain accurate medical records and contribute to a compassionate, home-like environment.

Qualifications

• Licensed Practical Nurse LPN license in good standing required
• BLS certification required
• Full-time day/evening shifts available

Responsibilities

• Evaluate and deliver care including medication administration and ADL care as ordered
• Conduct rounds to monitor residents and ensure quality care
• Communicate with residents, families, physicians, and supervisors about resident status

Licensed Practical Nurse LPN duties include but are not limited to:

• Evaluation and delivery of care including medication administration, treatments, Adl care, etc. as ordered by physician and outlined in the plan of care (according to scope of practice)


• Conduct regular rounds to monitor resident activity and ensure resident/patients receive quality care based on professional standards and facility policies


• Communicate with residents, family members, physicians and supervisor regarding resident status


• Maintain up to date and accurate medical records during tour of duty on assigned unit.


• Other tasks as required
