Job summary

Pleasant Hills Community Living Center is seeking a licensed Practical Nurse (LPN) to join our care team in a full-time role. You will provide compassionate nursing support, administer medications, and follow the plan of care to ensure residents receive quality service on our dedicated unit.

Qualifications

• Must hold a Licensed Practical Nurse (LPN) license in good standing.
• BLS certification required.
• Strong patient-care and communication skills essential.

Responsibilities

• Evaluate and deliver care including medication administration and treatments per care plan.
• Conduct rounds to monitor residents and ensure quality care.
• Communicate with residents, families, physicians and supervisor about status.
• Maintain accurate medical records during duty on assigned unit.
• Perform additional tasks as required by the unit.

Licensed Practical Nurse LPN duties include but are not limited to:

• Evaluation and delivery of care including medication administration, treatments, Adl care, etc. as ordered by physician and outlined in the plan of care (according to scope of practice)
• Conduct regular rounds to monitor resident activity and ensure resident/patients receive quality care based on professional standards and facility policies
• Communicate with residents, family members, physicians and supervisor regarding resident status
• Maintain up to date and accurate medical records during tour of duty on assigned unit.
• Other tasks as required
