Major Responsibilities:
  1. Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication reconciliation, monitoring vitals, medication administration, and documentation.
  2. Render and supervise care of clients on the unit.
  3. Monitor medication and consult with prescribers.
  4. Provide appropriate healthcare and coordinate referrals as prescribed by providers.
  5. Participate in the evaluation of client’s condition and progress with team members.
  6. Promote education and adherence to prescribed treatment protocols.
  7. Facilitate wellness and medically related psychoeducational groups to individuals.
  8. Record all client information within electronic medical record system daily.
  9. Assist RN on a daily basis.
  10. Offer nursing consult on emergency service evaluations.
  11. Work with clients to coordinate care with their PCP, psychiatrist, and other professionals, including medication refills and follow-up medical appointments.

Standard Staffing Level Responsibilities:

  1. Comply with established departmental policies, procedures, and objectives.
  2. Attend meetings, conferences, seminars as required or directed.
  3. Use Quality Improvement in daily operations.
  4. Comply with all health and safety regulations and requirements.
  5. Respect diverse views and approaches, demonstrate Standards of Respect, and contribute to a professional, tolerant, civil, and accepting environment.
  6. Maintain regular, reliable, and predictable attendance.
  7. Perform other duties as required or directed.

Position Qualifications:

License/Certification/Education:

  • Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in Massachusetts.
  • Satisfactory CORI background check.
  • Driving is not required.

Experience/Skills:

  • Proficiency in electronic documentation in compliance with grant, agency, state, and federal guidelines.
  • Effective and professional communication with clients, staff, and providers.

Preferred:

  • One year of nursing experience.
  • Knowledge of medical, substance abuse, and mental health nursing.

Equivalent education and experience may be substituted where appropriate.

Department-specific competencies will be developed and maintained in individual departments and reviewed regularly.
