Licensed Practical Nurse (LPN) – Bronson at Home

Location: BAH Bronson at Home. Position: Full Time, Days.

Job Overview

Under the provider’s direction, perform all general nursing staff functions for the practice, including recording vital signs, providing immunizations, initial patient assessment, obtaining concise patient history, and supporting providers. Manage chart entries, educate patients and families on medications, treatment and care, and facilitate referrals.

Responsibilities

• Work under provider guidance to perform all nursing staff functions.
• Record vital signs, immunizations, and patient histories.
• Prepare patients for examinations, including assessments, medication reconciliation, routine tests.
• Triage calls to determine urgency and appointment necessity.
• Assist provider with minor and emergency procedures.
• Schedule tests and procedures, provide instructions.
• Perform laboratory tests as directed and document results.
• Coordinate admissions, referrals, prescription calls.
• Take verbal, telephone, and written orders for medications.
• Distribute medications and record data accurately.
• Maintain and review patient records and charts.
• Participate in performance improvement monitoring and collect quality data.
• Communicate with referring providers and forward records as requested.
• Assist in completing required forms (Disability, Workers Compensation, State of Michigan).
• Attend required staff, educational, and other meetings.

Physical Demands

• Operations requiring sustained attention to computer or CRT for 40‑70% of time.
