Responsibilities

• Under the supervision of the RN, provide prescribed medical treatment and personal care services within the scope of practice of practical nursing to ill, injured, convalescent, and disabled persons as delegated, appropriate to the patient’s level of acuity and in accordance with established policies and procedures.
• Assist the Registered Nurse in collecting data, accompanying patients and responding to patient/family requests for services.
• Pass medications and administer IV therapy as required.
• Demonstrate competencies specific to the population served.
• Admit patients to the emergency department, complete preparation tasks, and coordinate with the charge nurse when patient needs exceed the scope of LPN practice or when census and acuity are high.
• Document all data gathered and care given; follow additional ProHealth guidelines when appropriate; communicate status of the patient to the physician.
• Maintain patient flow through the area.
• Implement skilled technical care within the limits of the LPN license and unit-specific and generic structure guidelines; implement appropriate protocols per physician order or in consultation with the charge nurse.
• Respond to call lights and monitor alarms, providing assistance to patients and families.
• Collect and record patient data including food and fluid intake and output, weight, vital signs, calorie counts, and demographic information.
• Manage IV fluid and medication administration; administer specified medication safely and appropriately, orally or by subcutaneous or intramuscular injection, and note time and amount on patients’ charts.
• Observe patients and report adverse reactions to medication or treatment to medical personnel in charge.
• Dress wounds, give enemas, douches, alcohol rubs, and massages; apply compresses, ice bags, and hot water bottles.
• Assist patients with bathing, dressing, and walking and turning.
• Collect samples (urine, blood, sputum) from patients for testing.
• Prepare or examine food trays for prescribed diets and feed patients.
• Wash and dress bodies of deceased persons.
• Assist in delivery, care, and feeding of infants.
• Assemble and use equipment such as catheters, tracheotomy tubes, and oxygen suppliers.
• Sterilize equipment and supplies.
• Inventory and requisition supplies.
• Utilize computerized systems to acquire needed supplies and to enter patient data as defined by role.
• Maintain a clean, orderly environment.
• Provide teaching materials to patients based on identified needs, guidelines for care, and in coordination with physicians and RNs; document completely.
• Follow through on the discharge process; assist patients with appointments for further care, rechecks, and others; use ProHealth guidelines and restrictions when appropriate; complete entries in ExpressCare log for each patient.
• Participate as a member of the health care team; develop and work toward achievement of yearly professional goals and objectives; meet organizational obligations professionally; precept new employees and students on the unit; communicate with the Registered Nurse about patient status, interventions completed, data collected, and pertinent input to facilitate care planning.

Qualifications

• High school diploma or GED and successful completion of an accredited Licensed Practical Nursing program.
• Experience in a health care delivery setting preferred.
• Licensed Practical Nurse in good standing with the State of Michigan.
• BLS certification by the end of the core orientation period.
• Ability to demonstrate specific clinical competencies required to perform the functions of the LPN within a general orientation period of approximately three months.
• Ability to move and respond quickly to emergency situations.
• Ability to input and retrieve data from computerized systems.
• Ability to rotate within service line and related service areas.
• Must effectively communicate with patients, visitors, and all members of the care delivery team; speech is clear, easily understood, and grammatically correct; writing is clear and legible, demeanor is comforting and positive.

Physical Demands

• Work involves very high levels of mental/visual fatigue (e.g., CRT work between 70% and 90% of the time).
• Requires constant standing or walking, regular lifting of light-weight objects (up to 20 pounds), and assisting with heavier tasks such as lifting and moving patients.
• Requires minor straining or fatiguing positions and some sustained hand/eye coordination.
