Senior Lifestyle Corporation is seeking a Licensed Practical Nurse (LPN) for Assisted Living in Cincinnati, Ohio. This full-time/PRN role covers 7:00 AM to 7:00 PM shifts, focusing on licensed nursing care for residents in accordance with state and federal regulations.

The ideal candidate holds an active LPN license in Ohio, CPR certification, 1+ year of senior care experience, and strong teamwork with good English communication.

Qualifications

• Must have an active Licensed Practical Nurse license in the practicing state.
• CPR certification required.
• English proficiency to perform duties and respond to emergencies.
• Minimum 1 year of experience in senior care or related field.
• Basic computer skills for documentation.

Responsibilities

• Administer medications in compliance with regulations and guidelines.
• Participate in admissions assessments and documentation.
• Develop and maintain acute and long-term care/service plans.
• Track clinical information such as wounds and infection control.
• Provide health-related consultations within the community.
• Notify physicians or family of health changes with proper documentation.
• Respond quickly to emergencies.
• Maintain communication with all departments and staff.
• Provide resident and staff educational meetings.
• Maintain confidentiality of resident information.
• Discharge planning, assessments, and clinical documentation as needed.

Job Description

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Qualifications

Level of Formal Education: Training from an accredited licenses or vocational nursing program
Area of Study: General nursing
Years of Experience: 1+ years
Type of Experience: Senior care or related field
Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.
