Job summary

The Palms at Sebring is seeking a compassionate Licensed Practical Nurse (LPN) for the 7pm to 7am shift to provide high-quality patient care in our skilled nursing facility.

You will collaborate with the interdisciplinary team, develop individualized care plans, and help monitor patient conditions to ensure optimal outcomes in a warm, supportive environment.

Qualifications

• Active LPN license in Florida.
• At least 2 years of experience (or equivalent).
• High school diploma or equivalent; additional education or certifications a plus.

Responsibilities

• Provide direct patient care, including medications, wound care, and vital sign monitoring.
• Develop and implement individualized care plans with the healthcare team.
• Conduct regular assessments and evaluations to identify and address patient needs.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Maintain accurate and up-to-date patient records, including charting and reporting.
• Participate in quality improvement initiatives to optimize patient outcomes and facility performance.
