Job summary

The Palms at Sebring is seeking a compassionate Licensed Practical Nurse (LPN) for the 7pm–7am shift to provide high-quality care in our skilled nursing facility. You will work with an interdisciplinary team to administer medications, monitor vital signs, and contribute to care plans.

Qualifications

Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Conduct regular assessments and evaluations to identify and address patient needs.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Maintain accurate and up-to-date patient records, including charting and reporting.
• Participate in quality improvement initiatives to optimize patient outcomes and facility performance.
