As a valued member of our healthcare team at The Palms at Sebring, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift from 7pm to 7am. In this rewarding role, you will be responsible for providing high-quality patient care, collaborating with our interdisciplinary team, and contributing to the overall success of our skilled nursing facility.
Provide direct patient care, including administering medications, wound care, and vital sign monitoring as needed
Develop and implement individualized care plans in collaboration with the healthcare team
Conduct regular assessments and evaluations to identify and address patient needs
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Maintain accurate and up-to-date patient records, including charting and reporting
Participate in quality improvement initiatives to optimize patient outcomes and facility performance

Active LPN license in the state of Florida
2 years of experience (or equivalent)--we believe in training and supporting our staff to excel in their roles
High school diploma or equivalent required; additional education or certifications a plus
