Job Summary

The Licensed Practical Nurse (LPN) will provide direct care to residents according to care plans, policies, procedures, and nursing standards.

Essential Job Functions
  1. Admit, transfer, and discharge residents, informing staff accordingly.
  2. Regularly check residents to observe and evaluate their physical and emotional status.
  3. Supervise Nursing/Resident Care Assistants and report concerns to the Resident Care Coordinator.
  4. Document care activities following policies, procedures, and regulations.
  5. Chart nurse’s notes clearly and comprehensively, reflecting care provided and resident responses.
  6. Report discrepancies and issues regarding resident care to the Resident Care Coordinator.
  7. Transcribe and verify physician orders for accuracy and execution.
  8. Administer and document medications as per physician’s orders.
  9. Complete assessments for all assigned residents each shift.
  10. Report residents’ conditions and shift events to incoming staff.
  11. Communicate pertinent clinical information with physicians, residents, families, and staff.
  12. Provide direct resident care as needed, adhering to care plans and standards.
  13. Perform additional duties and special projects as assigned.
Qualifications

Currently licensed as an LPN in Alabama. Preferably, one year of experience in a healthcare facility. CPR certification required within 30 days of employment.

Physical Requirements
  • Mobility throughout the workday.
  • Ability to reach, bend, and stoop as needed.
  • Good vision and hearing, or use of prosthetics, to meet job demands.
  • Ability to push, pull, or move at least 100 pounds with proper mechanics and assistance.
  • Stand or walk for approximately 75% of the day.
  • Ability to handle mental and emotional stress.
