Job summary

Hendrix Health and Rehabilitation is seeking an LPN to provide direct nursing care in our long-term care and rehabilitation setting. You will administer medications, monitor residents, assist with ADLs, and collaborate with RNs and therapists to support recovery goals.

Qualifications

• Current LPN license is required and active.
• Strong clinical skills including medication administration and wound care.

• BLS/CPR certification required.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health status, including vital signs, and report changes.
• Assist with ADLs such as bathing, dressing, and feeding as needed.
• Collaborate with rehab team to support recovery goals (PT/OT/SLT).
• Maintain accurate documentation per facility policies and regulations.
• Provide emotional support and education to residents and families.
• Ensure infection control and a safe, clean environment.

In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.

6. Provide emotional support and education to residents and their families.
7. Ensure a safe and clean environment, adhering to infection control protocols.
