Job summary

South Health and Rehabilitation LLC is seeking a Licensed Practical Nurse (LPN) to join our night shift team in Birmingham, AL. You will provide direct nursing care, assist with rehabilitation plans, and coordinate with RNs and physicians to ensure high-quality resident care in a long-term care setting.

Qualifications

• Current and valid LPN license.
• Strong clinical skills including medication administration and wound care.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status and vital signs; report changes to supervising RN/physician.
• Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding.
• Collaborate with rehabilitation team to support recovery goals (PT/OT/SLT).
• Maintain accurate documentation per facility policies and state/federal regulations.
• Provide emotional support and education to residents and families.
• Maintain a safe, clean environment following infection control protocols.

In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.
