Job summary

RHA Health Services, LLC is hiring a Licensed Practical Nurse (LPN) for the 2nd shift at the Hunter Ridge Intermediate Care Facility in Humboldt, TN. The role supports individuals with intellectual and developmental disabilities and mental health/substance use challenges, providing holistic nursing care and medication administration.

The position is full-time with a 4pm–12am schedule, requiring a valid nursing license, CPR certification, and ability to work with an Interdisciplinary Team.

Qualifications

• Graduation from an accredited licensed nurse program.
• Current nursing license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to obtain certification.
• Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.
• Must be able to work independently, be organized, and possess the ability to prioritize work tasks.

Responsibilities

• Provide holistic nursing care to individuals with developmental disabilities.
• Assist the Registered Nurse in delivering nursing services.
• Provide health related training to unlicensed personnel.
• Administer medications and treatments or assist with medication administration.
• Assist with ensuring preventative and routine medical appointments are scheduled.
• Conduct medication closest assessments.
• Collect health related data.
• Observe for and report any abnormal health related findings to the physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported.

Employment Type: Full-time

Compensation: $30.42

Incentives: No sign-on bonus offered. No relocation packaged offered.

Schedule: Monday - Friday,4pm - 12am

Job Responsibilities

Physical requirements include: ability to regularly lift 20 pounds. Must be able to lift a minimum of 40 pounds. Must be able to pull a minimum of 25 pounds. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to communicate.

• Education verification and other credentialing based on position requirements.
