Job summary

RHA Health Services, LLC seeks a Licensed Practical Nurse (LPN) for on-site night shifts in Trenton, TN. The role supports individuals with intellectual and developmental disabilities and those facing mental health or substance use challenges. You will work with an interdisciplinary team to provide comprehensive nursing care.

You will administer medications, educate staff, and help coordinate routine and preventive care. A valid LPN license and CPR certification are required.

Qualifications

  • Current nursing license with no restrictions to practice nursing.
  • CPR certification or eligibility to obtain certification.
  • Good communication with the Interdisciplinary Team and families.

Responsibilities

  • Provide holistic nursing care to individuals with IDD.
  • Assist RN in delivering nursing services to persons supported.

We are hiring for: Licensed Practical Nurse (LPN) - 12A - 8A Type: Regular

Location: Trenton, TN (On-Site) Employment Type: Full-time Compensation: $30.42 Incentives: No sign-on bonus offered. No relocation packaged offered. Schedule: Monday - Friday 12 AM to 8 AM

Job Responsibilities
  • Provide holistic nursing care to individuals with intellectual and developmental disabilities
  • Assist the Registered Nurse in delivering quality nursing services to persons supported
  • Provide health related training to unlicensed personnel
  • Administer medications and treatments or assist the person supported with medication administration
  • Assist with ensuring preventative and routine medical appointments are scheduled
  • Conduct medication closest assessments
  • Collect health related data
  • Observe for and report any abnormal health related findings to the physician
  • Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
  • Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported
Requirements
  • Graduation from an accredited licensed nurse program
  • Current nursing license with no restrictions to practice nursing.
  • Current CPR Certification or eligibility to obtain certification
  • Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families
  • Must be able to work independently, be organized, and possess the ability to prioritize work tasks
Physical requirements include:
  • ability to regularly lift 20 pounds.
  • Must be able to lift a minimum of 40 pounds.
  • Must be able to pull a minimum of 25 pounds.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop.
  • Must be able to regularly stand and walk.
  • Must be able to demonstrate proficiency in CPR from the floor level.
  • Vision requirements include close vision, distance vision, and peripheral vision.
  • Must be able to communicate.
