Job summary

The Pavilion at Jacksonville is seeking a Licensed Practical Nurse (LPN) for a part-time night shift (11 PM–7 AM) at our skilled nursing and rehabilitation facility. You will provide direct nursing care and supervise CNAs under regulatory guidelines and facility procedures.

Responsibilities include medication administration, wound care, resident assessments, and coordinating transfers as needed, with a focus on compassionate, outcomes-oriented care for residents.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable

Responsibilities

• Directs the daily activities of certified nursing assistants in accordance with regulations and policies.
• Promotes holistic care for residents using the nursing model.
• Ensures policy compliance by nursing staff.
• Assists in admission, discharge and transfers of residents as required.
• Monitors changes in residents’ status and notifies physician and family as needed.
• Transcribes physician orders to medical records and carries them out.
• Administers medications as prescribed and monitors for adverse effects.
• Performs wound care per orders and documents changes.
• Schedules follow-up appointments and transportation for residents.
• Provides nursing leadership to assigned staff.
• Other duties as assigned.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.
• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.
• Schedules follow up appointments for residents and transportation needs as indicated.
• Provides nursing leadership to nursing personnel assigned to the unit.
• Other duties as assigned.
