The Pavilion at Jacksonville is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care and supervise CNAs in a skilled nursing setting. Location: The Pavilion at Jacksonville. Job Type: Full-Time, Part-Time. Salary: Negotiable.

The role involves coordinating resident care, administering medications, and ensuring compliance with regulations. A valid LPN license and CPR certification are required, with LTC experience preferred.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable

Responsibilities

• Directs the day-to-day activities of certified nursing assistants in accordance with regulations and facility policies.
• Follows the nursing model to promote holistic resident care.
• Ensures policies and procedures are followed by nursing personnel.
• Participates in admissions, discharges and transfers as required.
• Assesses resident status changes, notifies physician and family, and documents accordingly.
• Transcribes physician orders to medical records and carries out orders as written.
• Prepares and administers medications as per orders and observes for adverse effects.
• Performs wound treatments as ordered and documents changes.
• Schedules follow-up appointments and transportation for residents.
• Provides nursing leadership to unit staff.
• Other duties as assigned.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.

• Follows the nursing model to promote holistic care for the residents.

• Ensures that policies and procedures are complied with by nursing personnel assigned.

• Schedules follow up appointments for residents and transportation needs as indicated.

• Provides nursing leadership to nursing personnel assigned to the unit.

• Other duties as assigned.
