CarePine Home Health is seeking a Licensed Practical Nurse (LPN) for Private Duty in Philadelphia, PA. Full-time or part-time positions with flexible schedules and pediatric/trach-vent care experience. Excellent pay up to $36/hour plus benefits.

Formación

• Active LPN license in Pennsylvania.
• CPR certification.
• Minimum two years pediatric care experience.
• Strong communication and organization skills.
• Compliance with professional standards.

Responsabilidades

• Provides direct patient care as defined in the State Nurse Practice Act.
• Implements a plan of care initiated by the clinician.
• Documents accurately and timely per the plan of care.
• Educates patient/family on diagnosis and care plan.
• Coordinates home health services and reports needs to the clinician.
• Uses equipment and supplies effectively and efficiently.
• Participates in personal and professional growth.
• Performs other duties as assigned by the RN.

Licensed Practical Nurse?(?LPN?)?Responsibilities

1.Licensed Practical Nurse?( LPN?) provides direct patient care as defined in?the State?Nurse Practice Act.?

2.LPN implements?a plan of care?initiated?by the?clinician.?

3\. Provides?accurate?and?timely?documentation consistent with the plan of care.?

4\. Licensed Practical Nurse?( LPN?) assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.?

5\. LPN?participates?in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the clinician and/or Clinical Supervisor.?

6\. LPN uses equipment and supplies effectively and efficiently.?

7\. Licensed Practical Nurse?( LPN?)?participates?in personal and professional growth and development.?

8\. Performs other duties as assigned by the Registered nurse.?

Licensed Practical Nurse?(?LPN?)?qualifications and?skills

• Currently licensed?as a?Licensed?Practical?Nurse in Pennsylvania?
• Valid?CPR Certificate.?
• At least two years experience working with pediatric patients.?
• Compliant?with?accepted professional standards and?practices.?
• Demonstrates good verbal and written communication, and?organizational?skills.?
