The role focuses on assessing, planning, implementing and evaluating individualized care plans, administering medications, and collaborating with a multidisciplinary team to ensure residents’ health, independence and well-being.

Qualifications

• Current Ohio LPN license/active licensure required.
• Ability to work evenings and adapt to shift schedules.
• Strong documentation and record-keeping skills.

Responsibilities

• Provide skilled nursing care, including assessment, planning, implementation and evaluation of care plans.
• Administer medications and treatments as prescribed by physicians.
• Take and monitor vital signs; report changes to the healthcare team.
• Conduct daily rounds to ensure comfort, safety and quality care.
• Collaborate with physicians, nurses and therapists to develop and implement comprehensive care plans.
• Maintain accurate and detailed records of resident care and treatment.

About the Role

As a Licensed Practical Nurse (LPN) on our evening shift, you will play a vital role in providing high-quality patient care to our residents.
