Job summary

The Avenue at Strongsville is seeking a compassionate Licensed Practical Nurse (LPN) to join our post-hospital rehabilitation and skilled nursing team. Night shift opportunities are available for full-time or part-time status in a state-of-the-art private-room facility.

As an LPN you will assess patients, administer medications, educate families, and collaborate with the care team to ensure individualized plans and seamless coordination of care in a family-owned environment with growth

Qualifications

• LPN license and compassionate patient care.
• Ability to assess and document patient condition, vital signs and history.
• Administer medications and therapies as prescribed by the medical team.
• Develop and implement individualized care plans.
• Collaborate with an interdisciplinary care team.
• Educate patients and families on condition and treatment options.
