Job summary

Aviata Health Group is seeking a dedicated Licensed Practical Nurse (LPN) in West Palm Beach to provide compassionate, person-centered care to residents and patients. Join a team focused on clinical excellence and community-minded service.

The LPN will support daily activities, participate in treatment plans, and help oversee falls and pressure sore initiatives. A flexible schedule including evenings, weekends, and holidays is required.

Qualifications

• Active LPN license in Florida (unencumbered).
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Ability to work evenings, weekends, and holidays.
• Strong communication and teamwork with residents, families, and staff.

Responsibilities

• Provide direct patient care to residents and patients under supervision.
• Assist in implementing individualized treatment plans.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Conduct evaluations of residents’ medical status at admission and during treatment.

About the Role

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team! As a Licensed Practical Nurse, you will play a vital role in providing exceptional, person-centered care to our residents and patients.

Responsibilities

• Provide direct patient care to residents and patients, assisting with activities of daily living under the direction of a professional nurse.
• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services and conduct thorough evaluations of each resident’s medical status upon admission and throughout their course of treatment.

Requirements

• A current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Ability to work a flexible schedule, including evenings, weekends, and holidays.
