Job summary

Chelsea Rehabilitation and Healthcare Center is seeking a Licensed Practical Nurse (LPN) to deliver compassionate care in a 84-bed sub-acute and long-term facility. You will work under the Marquis Health Consulting Services umbrella and provide hands-on care in collaboration with an interdisciplinary team.

The role emphasizes adherence to care plans, accurate documentation, and a resident-centered approach in a supportive, team-based environment.

Qualifications

• Current, unencumbered active LPN license in the state of VA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow care plans developed by the Nurse Manager and inter-disciplinary team.
• Perform medication administration and prescribed treatments per policy.
• Monitor residents, report changes promptly to supervising staff, respond to needs.
• Maintain accurate documentation per physician orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.
