LPN - Licensed Practical Nurse Team Leader - Position Overview

• Licensed Practical Nurses (LPNs) are essential members of our interdisciplinary team, managing the daily clinical regimen of each resident and sub-acute patient. Our LPNs gain extensive clinical experience and provide advanced care in a home-like setting.
• Direct nursing care of residents involves planning, developing, organizing, implementing, and evaluating resident care to ensure high-quality care and regulatory compliance.
• State LPN license required.

Responsibilities

• (LPN - Licensed Practical Nurse):
• Administer medications and treatments as ordered by a physician, utilizing the five rights.
• Monitor personal care duties and nursing procedures carried out by Nursing Assistants.
• Assist Nursing Assistants with direct resident care as needed.
• Report changes in resident condition, needs, and progress to the Unit Manager/Charge Nurse.
• Follow and monitor the Plan of Care for each resident.
• Manage storage and distribution of controlled substances securely.
• Document care activities accurately in care plans and records.

Qualifications

• Current state license as an LPN or RN required.
