Job summary

Monarch Healthcare Management is seeking an LPN in Minnesota to provide high‑quality direct routine care under nursing leadership and contribute to the Plan of Care for residents across Monarch's facilities.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• Current Certification in CPR (Cardiopulmonary Resuscitation).

Responsibilities

• Provide residents with direct routine care under nursing leadership.
• Participate in developing and modifying the resident Plan of Care.
• Implement delegated interventions ordered by RN or physician.
• Collect health data and report changes to RN promptly.
• Supervise and coordinate tasks of unlicensed nursing staff as needed.
• Support residents’ physical, mental, and psychosocial well‑being.

LPN- Licensed Practical Nurse

Eligible for $3,500 full time or $1,750 part time sign on bonus!!

Position Summary:

The LPN is responsible for providing residents with the highest quality of direct routine care, under the supervision of nursing leadership. The LPN participates in the development and modification of the tenant Plan of Care, implementing interventions that are delegated, ordered, or prescribed by the RN or physician. The LPN collects resident health data and compares to normal findings and the established current health status, reporting changes and/or responses to interventions in an ongoing manner to the RN. The LPN assigns and monitors nursing tasks or activities of unlicensed nursing staff through, assessment of their knowledge, skill, and capabilities, clarifying expectations, addressing behaviors, or escalating. All nursing activities are performed to support residents in achieving and maintaining the highest practicable physical, mental, and psychosocial well-being.

Qualifications:

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing Current Minnesota License as a Practical Nurse


• Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care
