Job summary

Monarch Healthcare Management is seeking a Licensed Practical Nurse (LPN) in Minnesota to provide high-quality resident care under RN supervision and in line with BON standards and the resident's plan of care. You will collect health data, report changes, and participate in QAPI processes.

You will work with skilled nursing and assisted living residents, coordinating tasks for licensed and unlicensed staff, while ensuring the highest practicable well-being for each resident.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing
• Current CPR certification
• Experience in skilled nursing and/or assisted living facilities

Responsibilities

• Provide residents with direct routine care under supervision of the RN and in accordance with Minnesota BON standards
• Assist in development and modification of the resident Plan of Care under RN guidance
• Monitor residents’ health data, report changes to the RN and escalate as needed
• Supervise and coordinate nursing tasks for unlicensed staff ensuring quality care

LPN - Licensed Practical Nurse

Position Summary:

The LPN is responsible for providing residents with the highest quality of direct routine care, under the supervision of the RN and in accordance with the Minnesota BON standards, the Resident's Bill of Rights and the facility Quality Assurance and Performance Improvement (QAPI) program. The LPN participates in the development and modification of the resident Plan of Care, implementing interventions that are delegated, ordered or prescribed by the RN or physician. The LPN collects resident health data and compares to normal findings and the established current health status, reporting changes and/or responses to interventions in an ongoing manner to the RN. The LPN assigns and monitors nursing tasks or activities of unlicensed nursing staff through, assessment of their knowledge, skill and capabilities, clarifying expectations addressing behaviors or escalating. All nursing activities are performed to support residents in achieving and maintaining the highest practicable physical, mental and psychosocial well-being.

Qualifications:

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing Current Minnesota License as a Practical Nurse


• Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care


• Experience in skilled nursing and/or assisted living facilities
