Slate Valley Center for Rehabilitation and Nursing in Granville, NY is hiring a Licensed Practical Nurse (LPN) to join our care team. Immediate hire with a $5,000 sign-on bonus and Daily Pay option.

Base rate ranges from $26 to $34 per hour, plus a $0.65 differential for evenings and nights, reflecting shift premium. The role includes collecting admission information, recording vitals, administering medications, wound care, supervising CNAs, assisting residents with daily activities, and

Qualifications

• Valid LPN State license.
• Must be in good standing with State Registry.
• Team-oriented with compassionate caregiving approach.

Responsabilités

• Collect information from new Residents to be admitted.
• Record health details, vitals, and temperatures.
• Administer medications and injections as needed.
• Treat wounds and dress bedsores.
• May supervise CNAs.
• Assist Residents with dressing and hygiene.
• Monitor Residents’ food and fluid intake and output.

DUTIES:

• Collecting required information from new Residents to be admitted.
• Recording health details of Residents; including vitals & temperature.
• Administering medications and injections to Residents as needed.
• Treating and dressing wounds and bedsores as needed.
• May be required to supervise Certified Nursing Assistants (CNAs).
• Helps Residents get dressed & take care of personal hygiene.
• Monitors Residents’ food and liquid intake and output.

REQUIREMENTS:

• Must be able to work as a team member.
• Valid LPN State license.
• Must be in good standing with State Registry.
