Job description

PRIMARY PURPOSE

To provide comprehensive, patient-centered nursing care within FoundCare’s Federally Qualified Health Centers (FQHC), supporting primary care delivery, chronic disease management, preventive services, and care coordination for diverse and underserved populations, including patients with complex medical and social needs, under the supervision of a Registered Nurse (RN) or provider.

ESSENTIAL JOB FUNCTIONS
Direct Patient Care
  • Perform patient intake, including vital signs, medical history updates, and medication reconciliation.
  • Administer medications, immunizations, and injections per provider orders and standing protocols.
  • Conduct point-of-care testing, including glucose, rapid tests, and urine dipstick.
  • Assist with procedures including wound care, EKGs, and minor office procedures.
  • Monitor patients and report changes in condition promptly to the provider and clinical team.
Care Coordination and Population Health
  • Support care management for patients with chronic conditions, including diabetes, hypertension, HIV, and Hepatitis C.
  • Assist in closing care gaps related to screenings, immunizations, and preventive services.
  • Conduct patient outreach and follow-up, including phone calls, appointment reminders, and care plan adherence.
  • Coordinate referrals, diagnostic testing, and specialty care as directed.
  • Participate in team-based care huddles to identify and prioritize patient needs.
Patient Education
  • Reinforce provider care plans and discharge instructions with patients and families.
  • Educate patients on medications, disease management, and preventive care.
  • Promote health literacy and self-management skills, with particular attention to high-risk populations.
Clinical Operations
  • Prepare exam rooms and ensure proper stocking of supplies and equipment.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR).
  • Ensure compliance with FQHC standards, UDS reporting measures, and clinical quality metrics.
  • Support infection control, OSHA, and patient safety protocols.
Team Collaboration
  • Collaborate as part of a multidisciplinary care team, including providers, RNs, Medical Assistants, behavioral health staff, and care coordinators.
  • Participate in quality improvement and performance improvement initiatives.
  • Contribute to Patient-Centered Medical Home (PCMH) model workflows and standards.
  • Promote the mission, vision, and values of FoundCare.
  • Maintain client and family confidentiality at all times.
  • Comply with all FoundCare policies and procedures.
  • Perform other duties as assigned.
Requirements
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
  • Working knowledge of practical nursing principles, clinical procedures, and chronic disease management.
  • Proficiency in Microsoft Office Suite, including Outlook, Excel, and Word, and ability to navigate electronic health record systems with accurate and thorough documentation practices.
  • Strong patient-centered communication skills with the ability to engage patients compassionately and clearly.
  • Cultural competence and sensitivity to the social determinants of health affecting underserved populations.
  • Ability to communicate clearly and professionally in English, both verbally and in writing, across all clinical and administrative interactions.
  • Ability to multitask accurately in a fast-paced clinical environment.
  • Strong attention to detail and documentation skills.
  • Ability to work collaboratively within a team-based care model.
  • Ability to handle difficult situations involving patients, providers, or others professionally.
  • Ability to maintain strict confidentiality of all patient, medical, and financial information in compliance with HIPAA regulations.
  • Ability to interact and work effectively with diverse populations.
  • Ability to review, understand, and apply concepts presented in training programs and professional literature.
  • Comfort working in an environment where staff education on HIV prevention and sexual health is part of the agency’s mission, which may include exposure to explicit health materials and direct conversations on sensitive topics.
PHYSICAL REQUIREMENTS
  • Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.
  • Full range of body motion required. Position requires reaching, bending, and handling objects with hands and/or fingers, talking and/or hearing, and sight.
  • Ability to lift and carry objects weighing 25 pounds or less.
  • Ability to wear personal protective equipment, including facemask, goggles, and latex or non-latex gloves, as required.
  • Ability to work in a patient care setting with possible exposure to blood and body fluids and communicable diseases, utilizing standard precautions to prevent disease transmission.
  • Use standard office equipment and technology, including computers, telephones, and clinical equipment and systems, in the course of daily duties.
  • Ability to travel to other FoundCare locations and perform job duties.
  • Ability to travel to other locations to attend meetings, workshops, and seminars, plus travel to other FoundCare departments and FoundCare conference rooms.
MINIMUM QUALIFICATIONS
  • Graduate of an accredited practical nursing program.
  • Active Licensed Practical Nurse (LPN) license in the state of Florida.
  • Current Basic Life Support (BLS) certification.
  • This role is not eligible for visa sponsorship.
PREFERRED QUALIFICATIONS
  • Experience in a primary care or community health care setting, particularly an FQHC.
  • Familiarity with chronic disease management, including HIV, Hepatitis C, and diabetes.
  • Knowledge of Epic or equivalent EHR system.
  • Bilingual proficiency in English and Haitian Creole or Spanish.
