Care Resource Community Health Centers, Inc. is seeking an experienced Licensed Practical Nurse (LPN) to join our Patient Care Team in South Florida. The role focuses on direct patient care, triage, pre-visit planning, and routine procedures under RN supervision.

Qualifications

  • 1 year of related work experience in primary care or similar setting.
  • Knowledge in promoting wellness and management of chronic diseases/conditions.
  • Training in HIV/AIDS is recommended.
  • Licensed Practical Nurse required (LPN).
  • BLS, OSHA certified preferred.

Responsibilities

  • Provide direct patient care and clinical support within the Patient Care Team.
  • Perform pre-visit planning and intake on patients with diverse problems.
  • Obtain EKGs, vision/hearing tests, and basic point-of-care testing; document results.
  • Chart and apply accurate charges for procedures/tests.
  • Support department operations, supplies, equipment, and inventory management.
  • Assist with training of support staff to ensure high quality care.
  • Promote patient-centered care and effective communication about wait times and needs.
  • Assist with medications, injections, immunizations, and vaccination clinics.

Care Resource Community Health Centers, Inc., is a Federally Qualified Health Center (FQHC) with four (4) locations located in Midtown Miami, Little Havana, Miami Beach, and Fort Lauderdale. We have been providing comprehensive health and support services for over 30 years in South Florida, to address the full health care needs of our pediatric, adolescent, and adult populations. In addition to those services, we provide comprehensive services to the community such as, Housing, Behavioral Health, Dental Care, and more! The Licensed Practical Nurse (LPN) supports the department by providing routine, direct patient care using evidence-based nursing practice. Acts as a clinical leader on the Patient Care Team to provide care with compassion, competence, and commitment.

Serves as a member of the Patient Care Team by providing direct patient care based upon nursing assessment and according to provider orders/protocol.

Performs pre-visit planning and intake on patients with diverse problems and medical histories.

Obtains electrocardiogram (EKG’s), vision and hearing tests, diabetic foot/monofilament tests, perform and charts POC tests. Documents & logs Maintenance of waived testing logs (blood glucose, A1C, strep, urine dipstick, urinalysis, pregnancy, Immunochemical Fecal Occult Blood Test (iFOBT), influenza A&B) and other tests as needed.

Charts and applies accurate charges for all procedures/tests.

Supports the Medical Department by improving the quality of care delivery, reducing the cost per visit and improving the Care Resource patient experience.

Assists with the management and maintenance of department’s supplies/materials/equipment and logs as needed.

Assists supervisor with daily oversight, purchase orders, and inventory of supplies/materials as needed.

Notifies supervisor when medical equipment needs repair or calibration.

Assists with the training of support staff (i.e. Patient Care Assistant, Laboratory Assistants, Receptionist/Medical Clerk) to ensure high-quality, and safe care.

Actively promotes patient-centered care by maintaining effective communication with patients about wait time, patient needs and expectations, and patient feedback.

Escorts patient to the different areas within the medical office (check-in/out, lab, exam room).

Completes all tasks sent via EHR by the requested timeframe.

Ensures compliance with the health center’s policies and procedures.

Provides nursing triage for overflow, emergency, isolation, code medical, and crisis intervention patients under the supervision of the RN if needed.

Assists with COVID vaccination clinic and outreach, administration of injections, and immunizations.

Participates in meetings, audits, site visits and supports other communication efforts with external contacts as required including Joint Commission and HRSA.

Prepares rooms for clinical examination and patient procedures as directed.

Ensures materials/supplies/equipment are fully stocked and available.

Assists Medical Providers with procedures/testing and ensures proper documentation in the medical records.

Performs high-level disinfection (HLD) and sterilization of instruments, as assigned and as per manufacturer's guidelines.

Quality Assurance/Compliance

Ensures examination rooms follow Occupational Safety and Health Administration (OSHA) and Joint Commission (JC) standards.

Ensures proper documentation/logging and disposal of medication into the hazardous/black box.

Receives training and undergoes competency assessments based on evidence-based guidelines and manufacturer's instructions for use.
